1. Always check whether you have the latest version of script:
http://www.forumadress.com/check_files.php
If not - you must update it.

2. Try to synchronize forums:
AP > Forum Admin > Synchronize
do it 2 times.

3. Check forums permissions, whether you have select private option on list.
set overall permissions of forums in Simple Mode to "REG".
AP > Forum Admin > Set Overall Permissions for all forums

4. Make sure whether you have some forums.
AP > Forum Admin > Management

5. If nothing helps:
Open database, look for table phpbb_forums
Browse it where is forum_id = 0, and delete this section or change forum_id to example 1000. Forum should work.
